Since it’s a very common problem that’s routinely looked for during general checkups of men over a certain age, odds are that he HAS gotten it checked out and is receiving treatment.
Unless he’s in a politically backwards country like the US and isn’t rich and/or one of those people too stubborn to go to the doctor.
Though I wouldn’t think that most people who would offer to donate a kidney (and have one healthy enough to be accepted) are very treatment averse…

Most men know, but the idea of your doctor giving you the finger isn’t very appealing.
